What You'll Do:

As a Hospice Social Worker, you will:

  • Conduct Comprehensive Assessments: Evaluate patients' social, emotional, and environmental needs to develop personalized care plans.​

  • Provide Counseling and Support: Offer emotional support and counseling to patients and their families, addressing end-of-life issues and facilitating coping strategies

  • Collaborate with Interdisciplinary Teams: Work alongside physicians, nurses, and spiritual care providers to ensure holistic care delivery.

  • Advocate for Patients: Ensure patients' needs and preferences are respected, acting as a liaison between patients, families, and healthcare providers.

  • Coordinate Community Resources: Assist patients and families in accessing necessary care and benefits by connecting them with community resources.​

  • Maintain Accurate Documentation: Keep detailed records of patient interactions and progress in compliance with regulatory standards.​

What You Need:

  • A Master's degree in Social Work (MSW) from an accredited institution is preferred.​

  • Must be a licensed social worker(LSW OR LCSW) in New Jersey.​

  • Minimum of two years in clinical settings, preferably in hospice, palliative care, or related fields.

  • Strong interpersonal and communication abilities, with a deep understanding of grief, loss, and the emotional aspects of end-of-life care.​
